Health screening

Seven questions adapted from the PAR-Q+, then a waiver. It takes about two minutes, it stays on this device, and nothing here is a medical assessment.

0 of 7 answered

Has a doctor ever said that you have a heart condition, or that you should only do physical activity supervised by a doctor?
Do you feel pain in your chest when you are at rest, or when you exert yourself?
Do you lose your balance because of dizziness, or have you lost consciousness in the last twelve months?
Have you been diagnosed with another chronic medical condition?
Are you currently taking prescribed medication for a chronic medical condition?
Do you have a bone, joint or soft-tissue problem that could be made worse by exercise?
Has a doctor ever told you to avoid exercise, or to exercise only under medical supervision, for any other reason?

Waiver

This waiver is placeholder copy. It has not been reviewed by a lawyer and it is not legal advice. If you own this site, have a qualified lawyer redraft and approve it before you publish or take a single sign-up.

I am taking part in this training program voluntarily. I understand that lifting loads and training under fatigue carries a risk of injury, and that no amount of coaching removes that risk entirely.

I confirm that the answers I gave in the screening above are true and complete as far as I know. I will tell the coach if my health changes while I am following the program.

I understand that this program is general fitness guidance written for a broad audience. It is not personal medical advice, it is not physiotherapy, and it is not a diagnosis or a treatment plan for any condition.

I understand that no specific result is promised. Outcomes depend on training, sleep, nutrition, stress, genetics and consistency, and they vary from one person to the next.

I agree to stop training and seek medical attention if I feel chest pain, unusual shortness of breath, faintness, or pain that is sharp rather than muscular.

I accept responsibility for my own decisions about what and how much to lift, and I release the coach and this site from liability for injury arising from my participation, to the extent permitted by the law where I live.

Placeholder clause. A lawyer will want to add the governing law, the parties named in full, how minors are handled, how long this record is kept, and any consumer rights that cannot be signed away in your jurisdiction.
